Volunteer Form

Due to logistical complications of scheduling individuals, World Hope International requests that volunteers align themselves with a group of 5 or more and select a team leader to correspond with personnel at the disaster site. The team leader is asked to complete the following volunteer form. Once the form is completed, someone from World Hope International’s Hope Corps office will contact the team leader to schedule a date for the team to serve. We appreciate your willingness to become involved in this way.
